Pitolisant (Wakix): Narcolepsy Uses, Safety and Interactions

What pitolisant does

Pitolisant is an oral wakefulness medicine acting at histamine H3 receptors. The EMA describes Wakix for narcolepsy with or without cataplexy in adults and children from age six. Cataplexy refers to episodes of muscle weakness associated with narcolepsy; treating daytime sleepiness does not guarantee normal alertness for driving.

In the reviewed UK Wakix label, use also includes excessive daytime sleepiness in selected adults with obstructive sleep apnoea when primary treatment has been inadequate or not tolerated. It does not open an obstructed airway: primary apnoea treatment remains important. That UK indication should not be assumed to appear in every country or generic product licence.

Common effects and individual assessment

Headache, insomnia and nausea are among the effects described in the European information. Specialist assessment is important because liver function, kidney function and other medicines can change exposure. The UK label contraindicates severe liver impairment and breastfeeding; pregnancy use requires weighing potential maternal benefit against fetal risk.

Heart rhythm and medicine interactions

The current reviewed US pitolisant-tablet label warns about QT prolongation and advises avoiding other QT-prolonging medicines and important rhythm-risk situations. Higher exposure with liver or kidney impairment can increase concern. Palpitations accompanied by fainting or collapse require urgent assessment.

Strong CYP2D6 inhibitors can raise exposure, whereas strong CYP3A inducers can reduce it. Centrally acting H1 antihistamines may oppose the intended wakefulness effect. Review prescription drugs, sedating allergy products and herbal medicines with the pharmacist rather than judging compatibility from drug names alone.

Contraception and limits of this summary

The US label warns that hormonal contraceptive effectiveness may be reduced and advises a non-hormonal method during treatment and for the specified period afterwards. The UK label likewise requires effective alternative contraception. Ask the prescriber to apply the exact current product instructions to the individual situation.

The US generic label reviewed here lists excessive daytime sleepiness in narcolepsy and explains that some branded paediatric information is omitted because of marketing exclusivity. Product-specific indication statements therefore matter. Keep follow-up focused on daytime functioning, adverse effects and the continuing need for treatment; this page supplies no titration or personal dosing plan.
