Augmentin 875/125 mg (Augmentin BID 1000 mg) and Warfarin (Coumadin 5 mg): interaction and safety

This source-based reference is not a personal treatment recommendation. The relevance and management of an interaction depend on the exact products, route, dose and clinical situation.

Interaction mechanism

Increased INR has been reported with amoxicillin and vitamin-K-antagonist anticoagulants; the mechanism and contribution of illness remain uncertain.

Clinical significance and safety

Augmentin 875/125 mg and Warfarin (Coumadin 5 mg): Amoxicillin/clavulanate may be prescribed with warfarin when appropriate, with additional INR monitoring after the antibiotic starts and stops. Infection, diet and clinical changes also influence anticoagulation. A small controlled trial in stable patients without infection did not reproduce the INR increase. The clinician should determine monitoring and adjustments. Separating doses does not replace INR monitoring.

Using this information

This is a source-based reference record, not a recommendation to combine, stop or change medicines. A missing interaction record does not establish safe use. A qualified healthcare professional should consider the exact products, doses, other medicines and individual health circumstances.
