Linaclotide: Uses, Safety and Drug Interactions

Linaclotide: uses, safety and drug interactions

Overview: Linaclotide is an intestinal guanylate cyclase-C agonist. It acts mainly inside the gut, increasing intestinal fluid and helping stool passage. This page uses the United Kingdom Constella label for adults with moderate to severe irritable bowel syndrome with constipation, rather than assuming that every country authorises the same indications or ages.

Uses and formulation: Constella is an oral capsule used after other organic causes of symptoms have been assessed. The UK product is taken before food; your own pack and prescription determine the appropriate regimen. It is not a treatment to start without assessment when bowel obstruction is possible. A clinician should reassess persistent symptoms and whether continued treatment remains useful.

Safety: Diarrhoea is the main adverse effect. Severe or prolonged diarrhoea can cause dehydration, electrolyte disturbance and dizziness. Blood in the stool or severe, persistent or worsening abdominal pain needs prompt medical assessment. The current UK label prefers avoiding treatment in pregnancy. It permits use during breastfeeding on the basis of very low absorption and a small milk study, while noting that effects on milk production have not been studied.

Drug interactions: Proton pump inhibitors, NSAID painkillers and other laxatives may increase diarrhoea risk. Severe or prolonged diarrhoea can reduce absorption of oral medicines, including contraceptive pills and levothyroxine. These are conditional precautions, not evidence that linaclotide directly blocks liver enzymes or that every combination causes harm. Ask a pharmacist or prescriber about additional contraception and monitoring when diarrhoea is significant.
