Rybelsus 7 mg (oral semaglutide): Uses, Formulations and Safety
Why the 7 mg formulation matters
Rybelsus is oral semaglutide for adults with type 2 diabetes. This page refers to the original 7 mg tablet recorded in this catalogue. The current US prescribing information still lists Rybelsus 3 mg, 7 mg and 14 mg tablets. Other markets have introduced more efficiently absorbed formulations with different strengths. A lower printed milligram amount therefore does not automatically mean a weaker clinical effect.
The UK regulator specifically warned about errors during the transition from oval 3/7/14 mg tablets to round 1.5/4/9 mg tablets. The current EU product information lists the newer strengths. These formulations are not interchangeable milligram for milligram, and taking extra tablets to imitate an old strength can cause an overdose. Product availability varies by country; a pharmacist should verify the exact dispensed pack before any change.
Use and absorption
Oral semaglutide is used alongside diet and exercise for diabetes management. It is not insulin and does not treat diabetic ketoacidosis. Absorption depends strongly on how the tablet is taken. The product-specific leaflet describes an empty stomach, a small quantity of plain water and a waiting period before food, drinks or other oral medicines. Tablets should be swallowed intact. Follow the instructions for the actual formulation rather than borrowing an injectable semaglutide schedule.
Interactions and important safety warnings
Nausea, abdominal discomfort, diarrhoea, vomiting and constipation can occur. Persistent symptoms can lead to dehydration and kidney injury. Severe continuing abdominal pain, a serious allergic reaction or sudden vision changes need prompt medical assessment. Inform the anaesthesia team before procedures because delayed stomach emptying can leave residual stomach contents.
Insulin and sulfonylureas can increase low-blood-glucose risk. Oral semaglutide can also alter exposure to other oral medicines; levothyroxine is a documented example, so thyroid monitoring may need review. Medicines with a narrow therapeutic range deserve particular attention. The US label contraindicates use with a personal or family history of medullary thyroid carcinoma or MEN2, advises stopping at least two months before a planned pregnancy, and does not recommend breastfeeding. Treatment changes require the prescriber; this summary provides no switching calculation.
