Saxenda (liraglutide): Weight Management, Safety and Interactions

Saxenda and the 6 mg/mL pen

Saxenda contains liraglutide, a GLP-1 receptor agonist used for weight management alongside dietary changes and physical activity. The 6 mg/mL figure describes the concentration in the pen, not the amount every patient should inject. This is a daily-injection product; instructions for weekly semaglutide or tirzepatide must not be applied to it. A clinician should assess eligibility and treatment response rather than continuing solely because some weight has been lost.

EU adult eligibility includes obesity, or overweight with at least one associated health condition such as hypertension or dysglycaemia. Paediatric authorisations have specific age, body-weight and obesity criteria which differ between regions: the current EU information includes selected children younger than those covered by the US label. Adult instructions must not be extrapolated to children, and this summary does not provide paediatric dosing.

Adverse effects and monitoring

Nausea, vomiting, diarrhoea and constipation are common. Ongoing vomiting or diarrhoea can cause dehydration and kidney injury. Severe persistent abdominal pain can signal pancreatitis and needs urgent assessment. Gallbladder problems, severe allergic reactions and a persistent increase in resting heart rate also require review. The US label contraindicates Saxenda in people with a personal or family history of medullary thyroid carcinoma or MEN2, based on animal tumour findings whose relevance to humans is uncertain.

Other medicines and procedures

Combining liraglutide with insulin or a sulfonylurea can increase hypoglycaemia risk; any medication adjustment belongs to the prescriber. Saxenda delays stomach emptying and may affect the handling of oral medicines. Report all prescriptions, over-the-counter products and weight-management preparations so the full regimen can be checked. The US label does not recommend combining Saxenda with other liraglutide products or any other GLP-1 receptor agonist. Tell the anaesthesia team before procedures because aspiration has been reported with this medicine class.

Pregnancy and breastfeeding

Weight-management treatment is inappropriate during pregnancy. Contact the prescribing clinician if pregnancy is planned or recognised so treatment can be stopped and care reorganised safely. EU information does not recommend liraglutide while breastfeeding because clinical experience is lacking. Never share the pen, even if its needle is changed; the exact device leaflet remains necessary for safe handling.
