Acalabrutinib: Uses, Interactions and Safety
Uses
Acalabrutinib is a Bruton tyrosine kinase inhibitor for selected chronic lymphocytic leukaemia and mantle cell lymphoma settings under current EU information. It may be used alone or with specified partners. The Turkish manufacturer capsule document reviewed describes previously untreated CLL alone or with obinutuzumab, and relapsed or refractory CLL after at least one therapy in patients without prior BTK inhibitor treatment; later EU extensions do not establish identical Turkish authorisation.
Administration
Take the exact capsule or tablet formulation prescribed and swallow it whole. Formulations must be distinguished when reviewing stomach medicines. Proton pump inhibitors can reduce absorption of the original capsule and should be avoided with that formulation. The newer maleate tablet has different acid-reducer compatibility. Antacids and H2 blockers require the capsule-specific timing plan supplied by the team.
Safety
Bleeding, serious or opportunistic infections, low blood counts and atrial fibrillation are important risks. Report fever, unusual bleeding, black stools, palpitations, fainting or new neurological symptoms promptly. Sun protection and checks for new skin lesions are recommended because second primary cancers have occurred. The team also assesses tumour lysis risk in susceptible patients.
Interactions
Strong CYP3A inhibitors can increase exposure and usually require avoidance or specialist interruption planning. Moderate inhibitors warrant monitoring under the EMA label and should not be automatically classified as strong inhibitors. Strong CYP3A inducers can reduce anticancer activity. Anticoagulants and antiplatelet medicines need a bleeding-risk assessment; vitamin K antagonists should not be coadministered under EMA information. Oral methotrexate needs spacing because of a potential intestinal transporter interaction. Venetoclax is an established partner in specified EU CLL regimens.
Pregnancy and breastfeeding
Pregnancy should be avoided. Treatment during pregnancy requires a specialist judgement when the clinical condition makes it necessary. Do not breastfeed during treatment or for two days after the last dose.
Adverse effects
Headache, diarrhoea, infections, bruising, nausea, fatigue and musculoskeletal pain are common. Keep blood-test appointments even when you feel well.
