Efavirenz and Maraviroc: interaction and safety

This source-based reference is not a personal treatment recommendation. The relevance and management of an interaction depend on the exact products, route, dose and clinical situation.

Interaction mechanism

Efavirenz induces CYP3A and decreases maraviroc exposure.

Clinical significance and safety

Efavirenz can reduce maraviroc exposure and compromise antiviral activity unless the HIV regimen is appropriately managed. The recommendation changes if a potent CYP3A inhibitor is also present, so the HIV prescriber must assess the complete regimen. Maraviroc is contraindicated with a relevant CYP3A inducer in patients with severe renal impairment or end-stage renal disease (creatinine clearance below 30 mL/min). Outside that restricted setting, the labels describe prescriber-directed regimen adjustment rather than a blanket prohibition. Do not independently change either medicine.

Using this information

This is a source-based reference record, not a recommendation to combine, stop or change medicines. A missing interaction record does not establish safe use. A qualified healthcare professional should consider the exact products, doses, other medicines and individual health circumstances.
