Strong CYP3A4 inhibition markedly increases voclosporin exposure.
Clinical significance and safety
Macrol 500 mg contains a systemic strong CYP3A4 inhibitor. The voclosporin label contraindicates this combination because of increased exposure and toxicity, including kidney toxicity. The treating team should select a safe alternative without the patient independently interrupting either prescription.
