Apixaban (Eliquis 5 mg) and Posaconazole: European product information does not recommend systemic strong inhibitors of both pathways, including posaconazole. The prescribing team should review an alternative or specialist plan. Uncontrolled bleeding, black stools or a severe new headache needs urgent assessment. Do not stop the anticoagulant yourself.
