It relieves symptoms; it does not identify or cure the cause of diarrhoea. This summary concerns oral 2 mg loperamide capsules, using the UK Imodium Classic label with additional NHS and US safety information. It does not cover combination products containing simeticone.

The UK product is for acute diarrhoea in people aged twelve and over and for diarrhoea episodes in adults with irritable bowel syndrome already diagnosed by a doctor. Age restrictions and prescription uses vary internationally. A child should not receive an adult product merely because the active ingredient has the same name.

Hydration and warning signs

Replacing lost fluid and electrolytes remains central to diarrhoea care. Loperamide cannot substitute for oral rehydration or treatment of an underlying infection. Frail adults and people with significant fluid loss need particular attention. NHS guidance advises medical review when nonprescription use is needed beyond forty-eight hours.

The reviewed UK label excludes use for bloody diarrhoea with high fever, certain invasive bowel infections, acute ulcerative colitis and antibiotic-associated pseudomembranous colitis. Marked swelling, constipation or suspected bowel blockage need urgent assessment. New or changing bowel symptoms should not simply be labelled as IBS.

Common effects and heart safety

Constipation, nausea, wind, dizziness or drowsiness can occur. Avoid driving if affected. Alcohol can increase unwanted effects. Taking more than directed can cause dangerous heart-rhythm disturbances, fainting, cardiac arrest or death. This warning is especially important with misuse, excessive exposure or interacting medicines; it is not a claim that every ordinary dose causes an arrhythmia.

Fast or irregular heartbeat, collapse or loss of consciousness requires emergency help. Tell a clinician about a previous rhythm disorder, long QT syndrome or low potassium.

Which medicines and personal circumstances matter?

Quinidine, ritonavir, itraconazole and gemfibrozil can increase loperamide exposure. Medicines that affect heart rhythm, and oral desmopressin, also warrant a specific check. The clinical significance of some pharmacokinetic findings at recommended doses remains uncertain; a raised blood level is not itself proof of harm in every person.

The cited UK label does not recommend use during breastfeeding and advises medical advice during pregnancy. Liver impairment can increase risk. Check the local leaflet and discuss the complete medicine list before self-treatment.

Sources

Imodium Classic 2 mg Capsules — UK SmPC — Text revised 21 April 2026; emc updated 6 May 2026

NHS — About loperamide — Reviewed 10 April 2024

MedlinePlus — Loperamide drug information — US National Library of Medicine patient information; checked 30 October 2026

Source check: 30 October 2026.
