The US oral capsule is indicated for dry mouth caused by Sjögren syndrome. It relieves a symptom rather than treating the underlying autoimmune disease. A Turkish university rheumatology text discusses sevimelin and notes local unavailability; that academic discussion does not establish Turkish marketing approval.

Administration

The capsules are taken by mouth following an individual prescription. Do not combine them with another saliva-stimulating medicine on your own. Treatment response, sweating and hydration should be reviewed, particularly in older people or those with several medical conditions.

Safety

Cevimeline is contraindicated with uncontrolled asthma, known allergy to the medicine and conditions in which pupil constriction is undesirable, including acute iritis or narrow-angle glaucoma. It can affect heart rate and conduction and may increase airway resistance and bronchial secretions. Controlled asthma, COPD and significant cardiovascular disease require close supervision. Gallstone and kidney-stone histories also matter. Excessive sweating can cause dehydration. Visual disturbance may impair driving, especially in dim light.

Interactions

Use with beta-blockers requires caution because of possible conduction disturbances. Other parasympathomimetic drugs, including pilocarpine and pyridostigmine, can produce additive effects on sweating, saliva, the bowel, heart and airways. Cevimeline may interfere with the desired action of antimuscarinics such as bladder medicines. Inhibitors of CYP2D6 or CYP3A can inhibit its metabolism; the label supports caution but does not quantify clinical risk for each individual combination. Tell the prescriber about all medicines rather than assuming a specific dose change.

Pregnancy and breastfeeding

Adequate controlled pregnancy studies are unavailable. Treatment should be considered only when expected benefit justifies potential fetal risk. Human milk transfer is unknown; a decision about breastfeeding or treatment should account for potential infant effects and maternal need.

Adverse effects

Excessive sweating, nausea, a runny nose and diarrhea are common treatment-related effects. Increased salivation also occurs. Seek urgent advice for wheezing, fainting, major pulse changes or dehydration. A persistent adverse effect warrants review of treatment.

Sources

Regulatory prescribing information (English)

Turkish medical or product information (Turkish)

Additional verified references

WHO ATC/DDD Index: cevimeline

FDA examples of drugs that interact with CYP enzymes and transporter systems

["post-12335","page","type-page","status-publish","hentry
["GET
