Uses<\/h2>\n

Macitentan is an endothelin receptor antagonist for long-term treatment of pulmonary arterial hypertension (PAH). It may be prescribed alone or with other PAH medicines. This summary follows the adult Turkish tablet indication. European authorisation also includes selected children and a separate dispersible formulation; those instructions should not be substituted for an adult prescription.<\/p>\n

Administration<\/h2>\n

The film-coated tablet is swallowed whole with water, with or without food. Treatment should be started and monitored by a PAH specialist. Keep follow-up appointments, since changes in symptoms, blood counts and liver tests can affect the treatment plan.<\/p>\n

Safety<\/h2>\n

Important concerns include liver injury, anaemia, fluid retention and pulmonary oedema in people with pulmonary veno-occlusive disease. Severe liver disease and substantial baseline liver-enzyme elevations prevent starting treatment. Report jaundice, severe fatigue, rapid weight gain or increased breathlessness promptly. The Turkish and European information recommend regular liver monitoring.<\/p>\n

Interactions<\/h2>\n

Strong CYP3A inducers can reduce effectiveness. Strong inhibitors and medicines inhibiting both CYP3A and CYP2C9 can increase exposure. Fluconazole and amiodarone need particular review; US labelling recommends avoiding these combinations, while European\/Turkish wording uses caution for several inhibitors. Sildenafil combination treatment has clinical evidence and is not automatically prohibited.<\/p>\n

Pregnancy and breastfeeding<\/h2>\n

Pregnancy and breastfeeding are contraindicated in the Turkish\/European information. Pregnancy must be excluded before treatment; reliable contraception and monthly pregnancy testing are needed. Avoid pregnancy for one month after stopping. Discuss pregnancy plans before changing treatment.<\/p>\n

Adverse effects<\/h2>\n

Headache, anaemia, upper respiratory infections and swelling can occur. New breathlessness or troublesome swelling needs assessment because the underlying disease may also cause these symptoms.<\/p>\n
