Uses<\/h2>\n

Palbociclib inhibits CDK4 and CDK6 and is used with endocrine therapy for hormone receptor-positive, HER2-negative advanced or metastatic breast cancer. The endocrine partner depends on prior treatment. Premenopausal or perimenopausal patients may also need ovarian suppression. Administration<\/h2>\n

Palbociclib is given in treatment cycles planned by the oncology team. Swallow the supplied capsules or tablets whole and follow the written calendar, including planned treatment-free periods. Capsules must be taken with food. Tablets have different food and stomach-acid medicine compatibility, so do not transfer capsule instructions automatically to tablets. Do not replace a vomited dose without advice.<\/p>\n

Safety<\/h2>\n

Low neutrophils and infections are major concerns, even without obvious symptoms between blood tests. Report any fever promptly. New or worsening cough or breathlessness may indicate lung inflammation and needs urgent assessment. Unilateral leg swelling, sudden chest pain or breathing difficulty can suggest a blood clot. The team may interrupt or modify treatment according to test results.<\/p>\n

Interactions<\/h2>\n

Strong CYP3A inhibitors increase palbociclib exposure, while strong inducers such as enzalutamide reduce it; these combinations should generally be avoided or managed by the specialist. Moderate inhibitors are not automatically contraindicated. Palbociclib can increase exposure to certain narrow therapeutic index CYP3A substrates. Severe muscle injury has been reported with simvastatin or atorvastatin: promptly report muscle pain, weakness or dark urine. Letrozole is an established treatment partner, not an interaction-based contraindication.<\/p>\n

Pregnancy and breastfeeding<\/h2>\n

Palbociclib is not recommended during pregnancy. Effective contraception is needed during treatment and for the label-specified period afterwards, which differs between women and men. Patients receiving palbociclib should not breastfeed. Discuss fertility concerns before treatment starts.<\/p>\n

Adverse effects<\/h2>\n

Common effects include neutropenia, infections, fatigue, nausea, mouth sores, anaemia, diarrhoea and hair thinning. Continue scheduled blood tests and tell the team about persistent symptoms.<\/p>\n
