It commonly contributes to the initial treatment phase and may also be included in selected drug-resistant TB regimens. It is not used alone, and the treatment team considers susceptibility results and previous therapy.

Administration

This summary concerns oral tablets, including the separate dispersible products used internationally. Follow the prescribed combination and follow-up schedule. Kidney disease may require a different interval under specialist supervision. Do not change the schedule or stop companion drugs independently when symptoms improve.

Safety

The principal safety concerns are liver injury and increased uric acid. Liver tests and uric acid are assessed before treatment and monitored as indicated. Severe liver damage and acute gout are contraindications. Persistent nausea, appetite loss, marked tiredness, dark urine or yellow skin require immediate contact with the treatment team. A hot, swollen, very painful joint also needs assessment.

Interactions

Pyrazinamide and isoniazid or rifampicin are deliberately combined in many regimens, but their liver toxicity can add up. This calls for monitoring, not a blanket prohibition. Probenecid has a complex two-way interaction and should generally be avoided. Allopurinol does not reliably treat pyrazinamide-related hyperuricaemia. Levofloxacin-containing combinations need careful adverse-effect monitoring. Avoid alcohol.

Pregnancy and breastfeeding

Pregnancy decisions balance limited drug-specific information against the substantial risk of untreated TB. The Turkish guide provides the local treatment context. Pyrazinamide enters breast milk in small quantities; WHO product information permits breastfeeding, whereas other product labels use more cautious wording. Milk exposure is insufficient to treat the infant.

Adverse effects

Joint pain, nausea, vomiting, poor appetite and raised liver enzymes are reported. Raised uric acid often produces no symptoms and is not, by itself, equivalent to an acute gout attack. Report symptoms so the clinician can distinguish a manageable laboratory change from a reason to change treatment.

Sources

Pyrazinamide tablets: US prescribing information

Pyrazinamide 150 mg dispersible tablets, TB307: WHOPAR Part 4

Turkish Ministry of Health tuberculosis diagnosis and treatment guideline (2026)

WHO ATC/DDD Index 2026: J04AK

["post-12399","page","type-page","status-publish","hentry
["GET
