Its approved uses differ by country. The US MYCOBUTIN label focuses on preventing disseminated Mycobacterium avium complex infection in advanced HIV. Turkish product information also describes selected MAC treatment and pulmonary tuberculosis uses, generally within an appropriate combination regimen.

Administration

Oral capsules can be taken with or without food according to the product instructions. The prescriber considers kidney function and interacting medicines when selecting treatment. Active TB must be excluded before using rifabutin alone for MAC prevention. Do not substitute it for rifampicin without the TB specialist’s plan.

Safety

Blood counts and liver tests may be monitored. Rifabutin can cause uveitis, particularly when interacting medicines raise its concentration. Eye pain, redness, light sensitivity or blurred vision require prompt assessment. Fever, unusual bruising, severe rash or jaundice should also be reported immediately.

Interactions

Rifabutin induces CYP3A and is itself affected by CYP3A inhibitors. It has important interactions despite often causing less induction than rifampicin. Clarithromycin and fluconazole can raise rifabutin exposure. Posaconazole has a two-way interaction; voriconazole and isavuconazole have major restrictions. Oral hormonal contraception may be less reliable, requiring an additional or alternative method. Other medicines may need blood-level or clinical monitoring.

Pregnancy and breastfeeding

Pregnancy and breastfeeding need specialist review. The Turkish label advises against use because clinical experience is inadequate, while the US label uses an individual benefit–risk approach. Do not assume these country-specific recommendations are identical.

Adverse effects

Rash, nausea, gastrointestinal discomfort and reduced neutrophil counts are reported. Urine, tears and other secretions may become orange-red, and soft contact lenses can stain permanently. This expected colour change should not obscure symptoms of liver injury. A negative pharmacokinetic study with a companion TB medicine does not establish universal combination safety.

Sources

MYCOBUTIN (rifabutin) capsules: US prescribing information

Turkish MYCOBUTIN product information

Turkish Ministry of Health tuberculosis diagnosis and treatment guideline (2026)

CRESEMBA: EMA product information, summary of product characteristics

["post-12405","page","type-page","status-publish","hentry
["GET
