It is used for serious ventricular arrhythmias and for maintaining normal rhythm in selected patients with atrial fibrillation or flutter. Indications vary between products. It has not been shown to improve survival in people with life-threatening ventricular arrhythmias.

Administration

This overview concerns oral tablets. Starting, restarting or increasing sotalol requires ECG assessment, kidney testing and correction of low potassium or magnesium. The US label requires initiation or reinitiation in a facility able to provide resuscitation and continuous ECG monitoring. Kidney function determines the dosing interval. Never stop it suddenly without medical direction.

Safety

Sotalol can prolong the QT interval and cause torsades de pointes, a potentially fatal rhythm. Fainting, new palpitations or collapse require emergency assessment. Slow heart rate, heart block, uncontrolled heart failure and asthma are major restrictions. Severe vomiting or diarrhoea can upset electrolytes and should be reported promptly.

Interactions

Other QT-prolonging drugs need specialist review; several combinations should be avoided. Amiodarone, flecainide and propafenone are not routine co-treatments. Diuretics and systemic corticosteroids can increase rhythm risk through electrolyte loss. Verapamil, diltiazem, digoxin and beta blockers may slow the heart further. Clonidine withdrawal needs a planned sequence. Some antacids must be separated from oral sotalol by at least two hours.

Pregnancy and breastfeeding

Use in pregnancy requires specialist consideration of maternal arrhythmia and fetal risks. Sotalol crosses the placenta and may slow the fetal or newborn heart or lower glucose. It reaches substantial breast-milk levels; the reviewed US label advises against breastfeeding during treatment.

Adverse effects

Fatigue, slow pulse, dizziness, breathlessness and gastrointestinal symptoms occur. These must be distinguished from dangerous proarrhythmia. Glucose monitoring may need extra attention with diabetes treatment because beta blockade can hide the usual rapid-pulse warning of hypoglycaemia.

Sources

Sotalol prescribing information

Turkish prescribing information

Sotalol tablets: Bristol Laboratories SmPC

Domperidone oral suspension: Zentiva SmPC

Sotalol rhythm treatment, QT and torsades risks, ECG and kidney monitoring, important interactions, and pregnancy and breastfeeding.

["post-12413","page","type-page","status-publish","hentry
["GET
