It is used in specified chronic lymphocytic leukaemia regimens and with other medicines for selected acute myeloid leukaemia patients. Combination partners and eligibility depend on the local label. The current EU CLL label includes specified ibrutinib and acalabrutinib combinations.

Administration

Swallow film-coated tablets whole with a meal and water. Starting, restarting and increasing treatment require a planned schedule with blood tests and tumour lysis prevention. Follow the team’s hydration and monitoring instructions. Do not restart after a treatment interruption without advice, and do not copy another patient’s escalation schedule.

Safety

Tumour lysis syndrome can develop rapidly and can cause kidney failure or dangerous electrolyte changes. Fever, chills, vomiting, dark or cloudy urine, reduced urine output, muscle cramps, unusual weakness or palpitations require urgent contact. Severe neutropenia and infection are additional major concerns. Blood counts and chemistry tests remain necessary after the initial period.

Interactions

Strong CYP3A inhibitors are contraindicated during CLL initiation and dose escalation. This restriction must not be extended indiscriminately to AML or CLL maintenance: those settings require regimen-specific specialist dose adjustment and close monitoring. Moderate inhibitors also need management. Strong or moderate inducers, including bosentan and modafinil, can reduce exposure and should be avoided. Warfarin requires closer INR monitoring. Narrow therapeutic index P-gp substrates need assessment, while bile acid sequestrants can reduce absorption. Avoid grapefruit, Seville oranges and starfruit.

Pregnancy and breastfeeding

Venetoclax can harm a fetus. Avoid pregnancy during treatment and for at least thirty days afterwards; hormonal contraception may require an added barrier method. Breastfeeding should stop during treatment. Fertility preservation may be discussed before treatment.

Adverse effects

Common effects include low neutrophils, anaemia, diarrhoea, nausea, vomiting, fatigue and infections, with differences between combination regimens. Promptly report fever or symptoms that prevent drinking fluids.

Sources

Venclyxto — EMA product information, Annex I SmPC

Turkish prescribing information: VENCLYXTO

Additional verified references

FDA examples of drugs that interact with CYP enzymes and transporter systems

Xtandi — EMA product information, Annex I SmPC

Erleada — EMA product information, Annex I SmPC

Kisqali — EMA product information, Annex I SmPC

PAXLOVID US prescribing information

["post-12427","page","type-page","status-publish","hentry
["GET
