Raltegravir

What raltegravir is used for

Raltegravir is an integrase strand transfer inhibitor used with other antiretroviral medicines to treat HIV-1. It is a component of a regimen rather than a stand-alone cure. Selection depends on prior treatment, viral resistance, age, weight and other clinical factors. Different formulations can have different eligibility and interaction recommendations.

Why the formulation is important

ISENTRESS film-coated tablets, chewable tablets, granules for oral suspension and ISENTRESS HD must not be substituted simply by matching printed milligrams. Their pharmacokinetic profiles differ. A recommendation for conventional film-coated treatment may not apply to the HD regimen, particularly when reviewing interactions. The prescriber or pharmacist should verify the exact product.

Antacids and enzyme-inducing medicines

Aluminum- or magnesium-containing antacids can reduce raltegravir absorption; the label does not recommend simultaneous or staggered administration. Calcium carbonate antacid advice differs between conventional ISENTRESS and ISENTRESS HD. Rifampin also requires formulation-specific specialist management. Strong inducers such as carbamazepine and phenytoin are not recommended because their effect is uncertain and reduced antiviral exposure is a concern.

Safety signals to recognize

Insomnia, headache, dizziness, nausea and tiredness are reported adverse effects. Severe skin or hypersensitivity reactions may involve fever, blisters, swelling or liver problems and require urgent assessment. Muscle injury, including rare rhabdomyolysis, has also been reported. Unexplained muscle pain, tenderness or weakness should be reported promptly, especially when other medicines can injure muscle.

Pregnancy and infant feeding

Pregnancy registry data do not show an increased overall birth-defect signal, but the amount of information differs by formulation. Pregnancy care should review the exact product rather than treating every regimen as equivalent. Infant feeding requires specialist HIV counseling; some labels retain advice against breastfeeding while current NIH guidance supports individualized shared decisions with viral suppression and monitoring.

Interpreting an interaction result

Studies found no clinically meaningful pharmacokinetic interaction for several specific combinations using conventional raltegravir. These results must not be generalized to every formulation or every patient. Maintain viral-load follow-up and an accurate medicines list, including non-prescription antacids and supplements. Do not interrupt HIV treatment or change formulation independently after reading a reference summary.
