Treprostinil

Treprostinil bir prostasiklin analoğudur. Bu kayıt PAH için kullanılan sürekli deri altı veya damar içi infüzyonu kapsar; pompanın kesintisiz çalışması ve kateter güvenliği tedavinin önemli parçalarıdır.
Ne İçin Kullanılır?
Türkçe TREPOKS KÜB’ünde NYHA sınıf III idiyopatik/kalıtsal PAH’ta semptom ve egzersiz toleransını iyileştirme.
Kullanılmaması Gereken Durumlar
Türkçe TREPOKS KÜB’üne göre aşırı duyarlılık; veno-oklüzif PAH; ağır sol ventrikül disfonksiyonuna bağlı dekompanse kalp yetmezliği; Child-Pugh C karaciğer yetmezliği; aktif gastrointestinal ülser/kanama veya intrakraniyal kanama; pulmoner hipertansiyonla ilgisiz miyokard bozukluğu yapan kapak hastalığı; ağır koroner hastalık/kararsız anjina; son 6 ayda enfarktüs, son 3 ayda inme/geçici iskemik atak; ağır aritmi; yakın gözetimsiz dekompanse kalp yetmezliği; başka prostanoidlerle kullanım.
Acil Değerlendirme Gerektiren Uyarılar
Pompa durması veya infüzyon kesilmesinde derhal tedavi ekibine ulaşın. Ani kesme PAH’ı kötüleştirebilir. Ateş/titreme, kateter enfeksiyonu, ciddi kanama, bayılma veya artan nefes darlığı acildir.
Treprostinil Gebelikte ve Emzirme Döneminde Kullanılır mı?
Gebelikte Kullanım
İnsan gebelik verileri sınırlıdır; PAH’ın kendisi anne ve fetüs için ciddi risk taşır. Gebelikte tedavi kararı uzman ekipçe verilmeli, infüzyon kendi başına kesilmemelidir.
Emzirme Döneminde Kullanım
İnsan sütüne geçiş bilinmiyor. Emzirmenin veya tedavinin sürdürülmesi anne-bebek yararı değerlendirilerek kararlaştırılır.
Treprostinil Kullanırken Alkol Alınır mı?
İncelenen ürün bilgisinde alkolle doğrudan etkileşim veya güvenli miktar tanımlanmamıştır; kişisel alkol kullanımı hastalık ve karaciğer durumuyla birlikte hekimle değerlendirilmelidir.
Treprostinil Yan Etkileri
İnfüzyon yeri ağrısı/reaksiyonu, baş ağrısı, ishal, bulantı, çene ağrısı, kızarma, ödem ve hipotansiyon.
