Treprostinil: Uses, Interactions and Safety
Uses
Treprostinil is a prostacyclin analogue that widens pulmonary and systemic blood vessels and inhibits platelet aggregation. This entry covers continuous subcutaneous or intravenous infusion for pulmonary arterial hypertension (PAH). The Turkish TREPOKS indication is narrower than the US Remodulin indication. Oral and inhaled products have different instructions and are not covered here.
Administration
An experienced team starts treatment, adjusts the infusion and teaches pump and catheter care. Never deliberately interrupt the infusion or make a large dose reduction yourself. Contact the treatment team immediately if the pump fails or delivery is interrupted; a restart may require supervised adjustment. Use only the prescribed equipment and preparation instructions.
Safety
Intravenous catheters can cause bloodstream infection and sepsis. Fever, shaking chills or catheter-site concerns require urgent review. Other important risks include low blood pressure, bleeding and deterioration after abrupt withdrawal. Turkish contraindications include several severe cardiac conditions, active bleeding and severe liver disease; US Remodulin lists no formal contraindications, so country-specific labels must be distinguished.
Interactions
Gemfibrozil and other CYP2C8 inhibitors may increase exposure; rifampicin and certain antiseizure inducers may reduce it. Much of the pharmacokinetic evidence comes from oral treprostinil, and the exact change during infusion is uncertain. Anticoagulants, antiplatelets and NSAIDs can increase bleeding risk. Diuretics and vasodilators can add to hypotension. These combinations need monitoring and are not all prohibited.
Pregnancy and breastfeeding
Pregnancy data are limited, and PAH itself makes pregnancy hazardous. Decisions require specialist assessment; do not abruptly stop an established infusion. Human milk data are lacking, so breastfeeding and treatment choices must be discussed.
Adverse effects
Infusion-site pain and reactions are particularly common with subcutaneous use. Headache, diarrhoea, nausea, jaw pain, flushing, swelling and low blood pressure can also occur.
