Ne İçin Kullanılır?
Nortriptilin, incelenen Birleşik Krallık tablet ürününde erişkinlerde majör depresif dönemlerin tedavisi için ruhsatlı bir trisiklik antidepresandır. Bu belge, ilacın bütün ağrı türleri veya uykusuzluk için onaylı olduğu anlamına gelmez; ülkeye ve endikasyona göre farklı kullanımlar ayrıca değerlendirilir. 18 yaş altındaki kişilerde bu ürün önerilmez.
Kullanılmaması Gereken Durumlar
Nortriptilin veya yardımcı maddelere aşırı duyarlılık; MAO inhibitörleriyle birlikte kullanım; yakın zamanda miyokart enfarktüsü, kalp bloğu, kalp ritim bozukluğu veya koroner arter yetersizliği; mani ve ağır karaciğer bozukluğu. İlaçlar arasındaki bekleme süreleri MAO inhibitörünün türüne göre değişir ve hekim tarafından planlanır.
Acil Değerlendirme Gerektiren Uyarılar
Bayılma, belirgin çarpıntı veya ritim bozukluğu, nöbet, yüksek ateşle birlikte huzursuzluk/titreme/bilinç değişikliği, ağır alerjik belirtiler veya yeni kendine zarar düşünceleri acil değerlendirme gerektirir. Brugada sendromu, dar açılı glokom, idrar retansiyonu ve epilepsi öyküsü özellikle bildirilmelidir. Aşırı alım ciddi kardiyak ve nörolojik toksisiteye yol açabilir.
Nortriptyline Gebelikte ve Emzirme Döneminde Kullanılır mı?
Gebelikte Kullanım
İncelenen Birleşik Krallık ürün bilgisi gebelikte ancak açıkça gerekli görüldüğünde ve yarar-risk değerlendirmesinden sonra kullanımı önerir. Geç gebelikte uzun süreli maruziyetten sonra yenidoğanda huzursuzluk, titreme, düzensiz soluma, beslenme güçlüğü veya antikolinerjik belirtiler gelişebilir. Tedavinin ani kesilmesi de sorun yaratabileceğinden plan hekim tarafından belirlenmelidir.
Emzirme Döneminde Kullanım
Nortriptilin anne sütüne geçer; ürüne ait tahmini maruziyet anne dozunun yaklaşık %0,6–1’idir. Düşük süt düzeyi bebeğe ilişkin riskin sıfır olduğu anlamına gelmez. Emzirmenin ve anne tedavisinin yararı birlikte değerlendirilerek emzirmeye veya ilaca devam kararı hekimle alınır.
Nortriptyline Kullanırken Alkol Alınır mı?
Alkol Risk Düzeyi: Yüksek
Nortriptilin alkolün merkezi sinir sistemi baskılayıcı etkisini artırabilir; etanol varlığında nortriptilin kan düzeyinde artış da bildirilmiştir. Alkolle birlikte kullanımdan kaçınılmalı; uyku hali veya dikkat bozukluğunda araç kullanılmamalıdır.
Nortriptyline Yan Etkileri
Ağız kuruluğu, kabızlık, bulantı, terleme, baş dönmesi, titreme, baş ağrısı, çarpıntı/kalp hızında artış, görmede odaklama güçlüğü ve kilo artışı sık veya çok sık bildirilen etkiler arasındadır. Ayağa kalkınca tansiyon düşmesi ve idrar yapma yakınmaları da görülebilir.
