Ziprasidone: Uses, Safety and Drug Interactions

Ziprasidone: uses, safety and drug interactions

Overview: Ziprasidone is an antipsychotic available as oral capsules. The United States capsule label reviewed here covers schizophrenia in adults, acute manic or mixed episodes of bipolar I disorder, and bipolar I maintenance as an addition to lithium or valproate. It does not establish a safe and effective paediatric capsule dose. Oral and injectable products have different instructions.

Uses and formulation: Take oral capsules with food and swallow them whole; do not open, crush or chew them. Food substantially affects absorption, so a prescription cannot be interpreted independently of this instruction. Treatment and switching schedules require a prescriber. Ziprasidone is not approved for dementia-related psychosis, and antipsychotic treatment in that older population carries an increased-mortality warning.

Safety: The heart-rhythm warning is particularly important. Existing long QT, a recent heart attack, uncompensated heart failure, and certain interacting medicines can rule out treatment. Fainting, palpitations, severe dizziness, fever with muscle stiffness, a severe rash or signs of serotonin syndrome require prompt assessment. Low potassium or magnesium adds risk. Sleepiness and movement-related effects can occur; avoid alcohol.

Drug interactions: Do not combine ziprasidone with contraindicated QT-prolonging medicines or MAO inhibitors. Other serotonergic medicines, sedatives and blood-pressure treatments need careful review. Carbamazepine can lower ziprasidone exposure, while CYP3A4 inhibitors can raise it. Pregnancy and breastfeeding decisions balance maternal treatment needs and infant risks. A pharmacist or prescriber should check the actual products and the whole medicine list before any change.
