Brexpiprazole: Uses, Safety and Drug Interactions

Brexpiprazole: uses, safety and drug interactions

Overview: Brexpiprazole is an atypical antipsychotic. The current European Rxulti information used for this page covers schizophrenia in adults and adolescents aged 13 years and older. Approvals for other conditions in another country should not be transferred automatically to this European product. The English active-substance name alone does not establish that every branded or generic presentation is interchangeable.

Uses and formulation: The film-coated tablets are taken by mouth, with or without food. A prescriber considers kidney and liver function, CYP2D6 metabolism and other medicines when planning treatment. Changes in interacting medicines can alter the dose requirement, including after an inhibitor or inducer is stopped. Do not adjust or replace treatment without a supervised plan.

Safety: Restlessness and weight gain are important adverse effects; nausea, sleepiness and dizziness can also occur. Seek prompt advice for fever with rigidity or confusion, fainting, serious allergic swelling, new suicidal thoughts, uncontrollable gambling or other new compulsive urges. Blood glucose, lipids, weight and movement symptoms may need monitoring. Use during pregnancy is not recommended under the reviewed European label; breastfeeding requires an individual benefit–risk decision.

Drug interactions: Strong CYP3A4 or CYP2D6 inhibitors can increase brexpiprazole exposure, while strong CYP3A4 inducers can reduce it. Combinations affecting both pathways require particular care. Alcohol and medicines with overlapping sedating effects, QT-prolonging medicines, electrolyte-depleting diuretics and medicines associated with raised creatine kinase also warrant review. These warnings do not mean that every combination is forbidden or that a specific harmful outcome has been demonstrated in every patient.
