Zofran (Ondansetron): Uses, Heart Safety and Interactions

What is Zofran used for?

Zofran contains ondansetron, a serotonin 5-HT3 receptor antagonist used to prevent certain types of nausea and vomiting. This page concerns the swallowed 4 mg tablet. It does not provide injection instructions or the handling instructions for orally disintegrating tablets. The reviewed US brand label includes prevention related to chemotherapy, radiotherapy and surgery.

The reason for vomiting matters. Ondansetron is not an automatic treatment for every episode of sickness and does not correct dehydration or remove the underlying cause. A clinician determines whether its use is appropriate, especially for children or people with liver disease. The brand and generic products available in different countries may have different approved uses.

Common effects and important warning symptoms

Headache and constipation are common. Diarrhoea, tiredness or flushing may also occur depending on the treatment setting. In people at risk of bowel obstruction, relief of vomiting can conceal worsening bowel problems. Severe abdominal swelling, persistent pain or reduced bowel activity needs assessment.

Ondansetron can prolong the QT interval, an electrical measurement of the heartbeat. The US label advises avoiding it in congenital long QT syndrome and considering ECG monitoring for relevant electrolyte abnormalities, heart failure, arrhythmias or other QT-prolonging medicines. Palpitations, fainting or chest pain require prompt medical attention. Severe allergy with swelling or breathing difficulty is an emergency.

Interactions that require an explicit check

Apomorphine must not be combined with ondansetron because profound low blood pressure and loss of consciousness have been reported. Other QT-prolonging medicines or medicines that disturb electrolytes can increase rhythm risk. Serotonergic medicines, including some antidepressants, have been associated with serotonin syndrome when combined with ondansetron.

New agitation, fever, sweating, rigidity or unusual muscle movements should be assessed urgently. Carbamazepine, phenytoin and rifampicin can lower ondansetron exposure. Small studies suggest that ondansetron can reduce the pain-relieving effect of tramadol; this is a specific interaction to discuss, not a reason to alter treatment independently.

Pregnancy, breastfeeding and product-specific advice

Advice differs between regional labels. The cited UK generic tablet label says ondansetron should not be used in the first trimester and advises against breastfeeding during treatment. Australian Zofran consumer information asks patients to check pregnancy and breastfeeding with their doctor. Discuss the exact product and indication with a clinician; do not treat historical pregnancy letter categories as a current safety guarantee.
