Alirocumab: Uses, Interactions and Safety
Uses
Alirocumab is a PCSK9-targeting monoclonal antibody that increases removal of LDL cholesterol from the blood. It can be used with a statin and other lipid-lowering treatment, or in selected patients unable to use statins. Current European information includes cardiovascular risk reduction in established atherosclerotic disease. The available Turkish KÜB has narrower, older indication wording, so international updates should not be mistaken for Turkish approval.
Administration
It is injected under the skin with the prescribed pre-filled device. After training, the patient or caregiver may administer it. Rotate sites and avoid injured or actively inflamed skin. Follow the device storage, preparation and disposal instructions. Lipid tests guide the clinician’s assessment of response.
Safety
Serious allergic reactions, including angioedema and hypersensitivity vasculitis, have been reported. Seek urgent help for swelling of the face or throat, breathing difficulty or a severe widespread reaction. Tell the prescriber about previous reactions before another injection. Severe liver or kidney impairment requires individual assessment because experience is limited.
Interactions
Statins, ezetimibe and fenofibrate can reduce measured alirocumab exposure by increasing target-mediated clearance. This did not eliminate the LDL-lowering effect in the studied treatment schedules. These are compatible treatment combinations, not contraindications or evidence that cholesterol treatment is failing. A biological medicine is not expected to produce conventional CYP-enzyme interactions, but all other medicines should still be reviewed.
Pregnancy and breastfeeding
Use in pregnancy is generally not recommended unless the clinical condition requires treatment. It is expected to cross the placenta. Breastfeeding is particularly discouraged during the colostrum period; later treatment and feeding choices also require individual discussion.
Adverse effects
Injection-site reactions, respiratory symptoms, itching and influenza-like illness are recognised effects. Report persistent or significant symptoms to the treatment team.
