Ethambutol: Uses, Interactions and Safety

Uses

Ethambutol is an oral medicine used with other effective drugs to treat tuberculosis. It helps protect the regimen against resistance and is not used alone for active TB. Drug-susceptibility results, kidney function, age and the overall treatment plan determine how it is used.

Administration

Tablets can generally be taken with or without food. Keep all appointments and follow the TB service instructions. Kidney impairment can increase exposure and may require the clinician to change the regimen. Aluminium-containing antacids reduce absorption; the US label advises avoiding them for at least four hours after ethambutol.

Safety

The major concern is optic neuropathy. Vision and colour discrimination should be assessed before and during therapy as appropriate. New blurred vision, difficulty distinguishing colours or a missing area of vision needs immediate contact with the TB team before another dose. Prompt recognition matters because permanent visual loss has been reported.

Interactions

The exact antacid matters: the interaction evidence concerns aluminium hydroxide, not every medicine used for indigestion. The combinations used in standard TB treatment are intentional. Concurrent medicines that also affect vision require a clinical review rather than an automatic conclusion that the TB regimen is prohibited.

Pregnancy and breastfeeding

The Turkish national guide allows necessary ethambutol use in pregnancy; US product wording asks the clinician to weigh benefit and possible fetal risk. Ethambutol passes into milk, and breastfeeding decisions depend on the clinical situation. Milk exposure does not replace the infant’s own preventive treatment if that is needed.

Adverse effects

Nausea, gastrointestinal discomfort, rash, joint pain and increased uric acid can occur. Liver injury is also reported. The older US label restricts use in children unable to report visual changes; this should not be substituted for current specialist childhood-TB guidance. Turkish and US tablet strengths also differ.
