Evolocumab: Uses, Interactions and Safety
Uses
Evolocumab is a monoclonal antibody that binds PCSK9, allowing the liver to remove more LDL cholesterol from the blood. It is used with diet and appropriate lipid-lowering treatment for selected patients with high cholesterol, including familial disorders. Cardiovascular risk-reduction indications vary by country; the Turkish information includes established cardiovascular disease with background statin treatment.
Administration
It is injected under the skin using the prescribed pen, syringe or other authorised device. A clinician may teach a patient or caregiver to give it at home. Rotate injection sites and follow the instructions for that exact device. Keep scheduled injections and lipid tests; symptom relief is not a reliable measure of cholesterol control.
Safety
A serious allergic reaction is the main urgent safety concern. Facial, tongue or throat swelling, breathing difficulty or a widespread severe reaction needs emergency care. Some device needle covers contain a latex derivative, while others do not. Tell the team about latex allergy and check the supplied presentation rather than assuming all devices are identical.
Interactions
Evolocumab is commonly combined with a statin, and sometimes ezetimibe, to achieve additional LDL reduction. Statins can increase its clearance, but the observed lipid response is preserved and this does not require a statin dose change solely because evolocumab is added. Evidence for other combinations is more limited; lack of a typical CYP interaction is not a guarantee of safety for every treatment.
Pregnancy and breastfeeding
Pregnancy experience is limited. Use is generally reserved for a clinical need established by the treating team. It is unknown whether it enters human milk, so breastfeeding and treatment benefits must be assessed together.
Adverse effects
Injection-site reactions, upper respiratory symptoms, influenza-like illness, back pain and joint pain may occur. Persistent troublesome symptoms should be reviewed.
