Linezolid: Uses, Interactions and Safety

Uses

Linezolid is an antibiotic for selected infections caused by susceptible Gram-positive bacteria, including some pneumonias, skin infections and vancomycin-resistant Enterococcus faecium infections. It does not treat Gram-negative infection. Its specialist use in drug-resistant TB regimens is addressed by TB guidelines and should be distinguished from the conventional product indications.

Administration

Treatment may be oral or intravenous. Oral products can generally be taken with or without food. Follow the exact suspension instructions and complete the planned course. Longer treatment, including TB regimens, requires additional monitoring rather than extrapolating the evidence from short antibacterial trials.

Safety

Blood counts are usually monitored weekly. Linezolid may cause low platelets or other blood-cell reductions, optic or peripheral neuropathy, lactic acidosis, hypoglycaemia and low sodium. New visual changes, unexplained bruising, persistent severe diarrhoea, muscle pain with dark urine, or repeated vomiting require prompt medical assessment.

Interactions

Linezolid reversibly inhibits monoamine oxidase. Other MAO-A or MAO-B inhibitors are contraindicated during treatment and within the preceding two weeks. Serotonergic antidepressants, triptans, buspirone and serotonergic opioids can cause serotonin syndrome. Management needs specialist coordination: Turkish labeling is more restrictive than the June 2026 US monitoring wording. Do not stop antidepressants yourself or delay emergency infection care. Agitation, fever, sweating, tremor, clonus or rigidity needs emergency help. Sympathomimetics can raise blood pressure.

Pregnancy and breastfeeding

Pregnancy requires an individual benefit–risk decision. Breastfeeding recommendations differ: Turkish product information advises interruption, while the US label discusses individual assessment and monitoring the infant for diarrhoea or vomiting. Discuss the applicable product guidance before treatment.

Adverse effects

Diarrhoea, nausea, vomiting and headache are common. Reduced blood counts and neuropathy become particularly important with prolonged exposure. Tell the team about diabetes medicines, diuretics and every psychiatric or cold remedy. Avoid excessive high-tyramine food or drink during treatment.
