Olmesartan: Uses, Interactions and Safety
Uses
Olmesartan medoxomil is an angiotensin II receptor blocker used for high blood pressure in adults and children aged six years and older. Blocking angiotensin II relaxes blood vessels. The tablet is a prodrug converted to active olmesartan after absorption. It is not a treatment for an acute hypertensive emergency.
Administration
This page concerns single-ingredient oral tablets. Take them consistently according to the prescription, with or without food. The Turkish tablet information advises swallowing with fluid without chewing. Selection of treatment depends on blood pressure, kidney function and volume status; dehydration can increase the risk of fainting.
Safety
Blood pressure, kidney function and potassium may need testing after initiation or treatment changes. Seek prompt assessment for markedly reduced urine, fainting or severe weakness. Severe persistent diarrhoea with weight loss can develop months or years after starting treatment and needs investigation for sprue-like enteropathy. Facial or throat swelling requires emergency care.
Interactions
Other renin–angiotensin blockers can increase kidney injury, high potassium and hypotension; dual blockade is generally avoided. Potassium-sparing diuretics need close potassium monitoring. NSAID painkillers may reduce the blood pressure response and impair kidneys. Lithium toxicity can occur. Sacubitril/valsartan already contains an ARB and should not be combined with olmesartan.
Pregnancy and breastfeeding
Do not use during pregnancy because medicines acting on this system can seriously injure or kill a developing fetus. Pregnancy requires prompt replacement under medical care. Breastfeeding is not recommended in the Turkish information; the US label calls for a feeding or treatment decision.
Adverse effects
Dizziness is the leading common reaction in the US trials. The Turkish information also lists headache, nausea, diarrhoea and fatigue. Persistent diarrhoea should receive medical assessment instead of being assumed to be an ordinary mild side effect.
