Ramelteon: Uses, Safety and Drug Interactions

Ramelteon: uses, safety and drug interactions

Overview: Ramelteon acts at melatonin receptors and is used for insomnia characterised by difficulty falling asleep. This summary follows a July 2026 US generic-tablet label. It does not establish that ramelteon is suitable for every sleep complaint or that products available in different countries have identical approved uses and instructions.

Uses and formulation: The medicine is taken by mouth before bedtime under an individual prescription. A high-fat meal can delay its effect, and hazardous activities should be avoided after taking it. Persistent or worsening insomnia needs evaluation for an underlying condition rather than repeated self-adjustment. Safety and effectiveness in children have not been established.

Safety: Sleepiness, dizziness, fatigue, nausea and worsening insomnia can occur. Severe allergy involving the tongue or throat requires emergency assessment. Unusual sleep activities, hallucinations, behavioural changes or suicidal thoughts need prompt medical review. Severe liver impairment and severe sleep apnoea are important restrictions. Pregnancy and breastfeeding require a benefit-risk discussion; available data do not justify a blanket claim of safety.

Drug interactions: Ramelteon must not be taken with fluvoxamine, which markedly increases ramelteon exposure. Rifampin can reduce its effect. Fluconazole, strong CYP3A inhibitors, donepezil and doxepin may increase exposure and require appropriate supervision. Do not combine it with alcohol. Other CNS depressants may increase impairment or complex sleep behaviours, so provide a complete medicine list before treatment is prescribed.
