Nimodipine: Uses, Interactions and Safety

Uses

Nimodipine is a calcium channel blocker used after subarachnoid haemorrhage caused by a ruptured intracranial aneurysm. It improves neurological outcome by reducing ischaemic deficits; it is not a general headache remedy. This summary primarily follows the US Nymalize oral solution, alongside Turkish oral tablet information.

Administration

The oral solution is administered by mouth or an appropriate nasogastric/gastric tube only. Never inject an oral nimodipine product: wrong-route administration can be fatal. Turkey has a separately formulated intravenous Nimotop product used in hospital; that does not make an oral product injectable. Oral Nymalize is given away from meals on a closely scheduled hospital plan.

Safety

Blood pressure and pulse must be watched carefully. Cirrhosis can increase exposure and requires a different prescribed regimen. Tell the treating team immediately about faintness or neurological deterioration. The Turkish tablet information describes treatment following a separate infusion course; this is a country/formulation difference, not a universal sequence for all products.

Interactions

Strong CYP3A inhibitors, including clarithromycin, itraconazole, posaconazole, voriconazole and ritonavir, can cause substantial hypotension and are generally avoided. Fluconazole, diltiazem and verapamil have a lower inhibitory strength but still require monitoring. Strong enzyme inducers can reduce efficacy; rifampicin, carbamazepine and phenytoin are contraindicated with the Turkish oral tablet. Other blood-pressure medicines and PDE5 inhibitors can add to hypotension. Avoid grapefruit.

Pregnancy and breastfeeding

Human pregnancy data are insufficient and animal developmental harm has been observed. Specialists must balance the severity of the haemorrhage with fetal risk. Nimodipine enters human milk. Turkish information advises against breastfeeding during treatment; the US label calls for an individual benefit–risk decision.

Adverse effects

Low blood pressure is the main adverse effect. Headache, nausea, slow pulse and gastrointestinal symptoms are also reported. The clinical team should review new symptoms because the underlying haemorrhage itself can produce important changes.
