Suvorexant: Uses, Safety and Drug Interactions

Suvorexant: uses, safety and drug interactions

Overview: Suvorexant is an orexin receptor antagonist used for insomnia involving difficulty falling asleep, staying asleep, or both. The information here follows the US Belsomra prescribing information. Suvorexant is the active substance name; it should not be taken as proof that an unbranded generic product is marketed in every country.

Uses and formulation: It is an oral medicine for adults and should be considered after possible medical or psychiatric causes of sleep disturbance are assessed. A full night of sleep must remain before planned awakening. The prescriber selects the regimen, especially when other medicines affect suvorexant exposure. Different tablet strengths are not interchangeable without checking the prescription.

Safety: Daytime sleepiness, impaired driving and falls can occur even when the medicine is used as prescribed. Narcolepsy is a contraindication. Sleepwalking, sleep-driving or other activities performed without full awareness require immediate medical attention; the US label directs discontinuation if these occur. New suicidal thoughts or concerning behavioural changes also require assessment. Pregnancy, breastfeeding and significant respiratory illness need individual review.

Drug interactions: Alcohol and other CNS depressants increase impairment, and using another insomnia medicine with suvorexant is not recommended. Strong CYP3A inhibitors such as itraconazole, clarithromycin and ritonavir should generally not be combined with it. Moderate inhibitors require prescriber-led adjustment. Rifampin, carbamazepine and phenytoin can reduce efficacy. Digoxin concentrations should be monitored when the medicines are used together.
