Silodosin: Uses, Interactions and Safety

Uses

Silodosin is an alpha-1 blocker for urinary symptoms caused by benign prostatic hyperplasia in adult men. Relaxation of smooth muscle around the prostate and bladder outlet can improve urine flow. It is not a treatment for high blood pressure, and assessment is needed to distinguish benign enlargement from other causes of urinary symptoms.

Administration

The medicine is taken by mouth with a meal. Follow the instructions for the exact capsule supplied. Turkish UROREC information directs swallowing the capsule whole; some US products permit specific administration with applesauce. These instructions should not be transferred between products without checking. Kidney function can change the prescribed regimen.

Safety

Dizziness, postural hypotension and occasional fainting may occur, especially when treatment begins. Rise carefully and seek help for severe symptoms. Tell the eye surgeon about current or previous silodosin before cataract surgery because intraoperative floppy iris syndrome can complicate surgery. Severe kidney and liver impairment are contraindications in the US label, while the examined Turkish document describes use as not recommended.

Interactions

Strong CYP3A4 inhibitors can substantially raise silodosin concentrations. The US label contraindicates combinations with strong inhibitors such as itraconazole and ritonavir. Moderate inhibitors such as diltiazem or verapamil require adverse-effect monitoring. Ciclosporin is not recommended because it inhibits P-glycoprotein. Other alpha-blockers should not be added. Sildenafil, tadalafil, vardenafil or avanafil can add to blood-pressure lowering; a prescriber should review treatment and symptoms. Antihypertensive treatment may also warrant closer observation.

Pregnancy and breastfeeding

The reviewed products are intended for men with benign prostatic hyperplasia and have no indication for pregnant or breastfeeding women.

Adverse effects

Ejaculation changes, including reduced semen or retrograde ejaculation, are prominent recognized effects and can affect fertility temporarily. Dizziness, diarrhea, nasal congestion and orthostatic hypotension also occur. Report bothersome effects rather than changing the prescription yourself.
