Sulpiride: Uses, Interactions and Safety

Uses

Sulpiride is a benzamide antipsychotic. Turkish Dogmatil 200 mg is used for acute and chronic psychoses; the reviewed UK tablet label specifies acute and chronic schizophrenia. Different low-strength or liquid products may have different instructions, so the actual prescription and country should guide use.

Administration

Take the oral tablet as prescribed. Kidney function is important because sulpiride is mainly eliminated through the kidneys and treatment may require adjustment. Take sulpiride at least two hours before an antacid or sucralfate because these products reduce absorption.

Safety

Sulpiride can raise prolactin, cause movement disorders and prolong QT. An ECG and review of electrolytes and other medicines may be needed before treatment. The Turkish label includes restrictions for Parkinson disease, seizures, existing hyperprolactinemia and breastfeeding in addition to prolactin-dependent tumors, pheochromocytoma and acute porphyria. Its pediatric restrictions are also more detailed than the UK tablet label. These country differences should not be flattened into one universal contraindication list.

Interactions

The label advises avoiding concurrent antipsychotics unless a specialist plans an exception or a supervised transition. Levodopa and antiparkinson medicines including ropinirole are contraindicated combinations because their effects oppose each other. Other QT-prolonging drugs should generally be avoided. Bradycardia-inducing medicines and potassium-depleting diuretics increase rhythm risk and need assessment. Alcohol and other sedatives increase impairment. Lithium can increase movement symptoms and neurotoxicity; antihypertensives and nitrates can increase postural hypotension.

Pregnancy and breastfeeding

Pregnancy is generally not recommended unless expected benefit justifies risk. Third-trimester exposure can cause newborn movement or withdrawal symptoms. Sulpiride passes into milk; the Turkish product contraindicates breastfeeding during therapy, while the UK label calls for an individualized feeding-versus-treatment decision.

Adverse effects

Sleepiness, insomnia, constipation, weight gain and prolactin-related symptoms may occur. Seek urgent care for fainting, palpitations, fever with stiffness and confusion, severe allergy or infection symptoms.
