Trifluoperazine: Uses, Interactions and Safety

Uses

Trifluoperazine is a phenothiazine antipsychotic used for schizophrenia and other psychoses. Some oral labels also include short-term adjunctive treatment of anxiety-related symptoms or agitation, and symptomatic nausea and vomiting treatment. These uses do not make it a general-purpose long-term anxiety or sleep medicine.

Administration

This summary concerns oral tablets. The indication, duration and any taper should be agreed with the prescriber. Benefits and adverse effects should be reviewed periodically. Do not substitute an injection or liquid without checking the specific formulation instructions.

Safety

Trifluoperazine can cause restlessness, muscle spasms, parkinsonism and persistent involuntary movements. Older adults may be more sensitive to hypotension and other effects. Tell the clinician about blood disorders, heart failure, liver disease, glaucoma, urinary problems and epilepsy. Turkish labeling contraindicates uncontrolled heart failure, blood disease, liver failure and coma. Dementia-related psychosis carries the class warning of increased mortality.

Interactions

The Turkish label advises avoiding concurrent neuroleptics unless a specialist plans a supervised exception. Alcohol, opioids, hypnotics and other sedatives increase impairment. Antimuscarinic medicines can worsen dry mouth, constipation and urinary retention. Levodopa’s benefit can be reduced. Lithium requires exceptional caution because severe movement effects and neurotoxicity have been reported. Warfarin effectiveness may change, requiring INR monitoring. Antacids can reduce oral absorption. Combining QT-prolonging or electrolyte-depleting medicines raises ventricular arrhythmia risk and requires careful assessment; the reviewed label does not declare every such pair contraindicated.

Pregnancy and breastfeeding

Pregnancy use is reserved for situations in which expected benefit justifies potential risk. Third-trimester exposure can cause newborn movement or withdrawal symptoms. Trifluoperazine enters breast milk, so a feeding or treatment decision is needed.

Adverse effects

Report new involuntary movements or severe restlessness promptly. Fever with rigidity and confusion, fainting, marked palpitations or signs of infection require urgent medical evaluation. Drowsiness and dizziness can make driving unsafe.
