Trospium: Uses, Interactions and Safety
Uses
Trospium chloride is an antimuscarinic medicine for overactive bladder, including urgency, frequent urination and urge incontinence. This page covers single-ingredient immediate-release tablets. It does not cover the xanomeline/trospium combination used for another condition or assume that extended-release capsules have identical instructions.
Administration
Take the prescribed tablet by mouth on an empty stomach or before food according to the product leaflet. Food can markedly reduce absorption. The reviewed US tablets contain 20 mg, whereas Turkish SPASMEX contains 30 mg; instructions and regimens should not be exchanged. Kidney impairment and older age may require a different prescription.
Safety
Urinary retention, gastric retention and uncontrolled narrow-angle glaucoma are contraindications in the US label. Turkish SPASMEX information has additional restrictions, including myasthenia gravis, certain bowel disorders, fast irregular heart rhythms and dialysis-dependent kidney disease. It also instructs patients with severe liver disease not to take SPASMEX. Inform the prescriber about glaucoma, obstruction, kidney disease or severe constipation. Angioedema can occur even after the first dose; tongue or throat swelling or breathing difficulty needs emergency treatment. Reduced sweating can cause heat illness.
Interactions
Other antimuscarinic medicines can increase dry mouth, constipation, urinary retention and related effects. Trospium can change gastrointestinal movement and therefore the absorption of other medicines. Some drugs sharing renal tubular secretion pathways may require closer monitoring. A metformin interaction was studied with an extended-release formulation; its numerical result should not be applied directly to these immediate-release tablets. The Turkish leaflet advises avoiding alcohol if possible; alcohol can worsen drowsiness and impair driving.
Pregnancy and breastfeeding
Adequate human pregnancy evidence is lacking, and treatment requires a clinician to weigh benefit against potential risk. Passage into human milk is unknown; discuss breastfeeding and the need for treatment with the prescriber.
Adverse effects
Dry mouth, constipation, indigestion, headache and abdominal discomfort are recognized effects. Seek prompt advice for inability to urinate, severe constipation, confusion or heat-related illness. Do not drive while drowsy or visually affected.
