Zuclopenthixol: Uses, Interactions and Safety

Uses

Zuclopenthixol is a thioxanthene antipsychotic used for psychoses, particularly schizophrenia. Turkish oral Clopixol labeling includes symptoms such as agitation, restlessness, hostility and aggression alongside psychosis. It is not a routine medicine for dementia-related behavioral changes.

Administration

This page is based on oral film-coated tablets, swallowed with water. The acetate injection used for short-term acute treatment and the decanoate depot used for maintenance have different schedules and conversion requirements. A clinician must plan any overlap and reduction when changing from tablets to a depot; patients should not calculate an equivalent dose themselves.

Safety

Drowsiness, hypotension and movement disorders are important monitoring points. Discuss seizures, cardiac rhythm disease, low potassium or magnesium, glaucoma, urinary obstruction, breathing disease and liver or kidney impairment. Circulatory collapse, coma and depressed consciousness from intoxication are contraindications. Use in children is not indicated because clinical experience is insufficient. Older people are especially sensitive to sedation, confusion and temperature changes.

Interactions

The label advises avoiding concurrent antipsychotics unless a specialist plans an exception or a supervised transition. Alcohol and other sedatives increase impairment. Antimuscarinic medicines can increase constipation or urinary retention, and metoclopramide can worsen movement effects. Lithium should generally be avoided because of neurotoxicity and QT concerns. QT-prolonging medicines such as sotalol, amiodarone or moxifloxacin should generally be avoided; electrolyte-depleting diuretics require caution. CYP2D6 inhibitors can raise zuclopenthixol exposure. Some antihypertensives increase postural hypotension, and levodopa’s benefit may be reduced.

Pregnancy and breastfeeding

Pregnancy treatment requires an individualized decision. Late exposure can produce newborn movement, withdrawal, feeding or breathing difficulties. Small amounts enter milk; continued breastfeeding, when clinically justified, needs a clinician-agreed observation plan.

Adverse effects

Sleepiness, dry mouth, akathisia, tremor, constipation and dizziness may occur. Seek urgent help for fever with muscle rigidity and confusion, fainting, severe palpitations or new difficulty swallowing or breathing.
